When you visit our office for the first time we will need you to fill out a somewhat extensive set of forms to make sure we have all pertinent data on your heath, and to insure you are aware of our office policies and your rights as a patient.
Please contact our office and we will send you a HIPAA secure on-line form to be filled out before arriving for your appointment. If there is something you don’t understand, you are welcome to leave it blank and ask our front desk staff when you arrive. Please note: You do not have to print out the HIPPA Notice PDF, it is for your information only and does not need to be brought into the office.
